Elite Health & Hormone is a private, cash-pay telehealth medical practice. This Financial Policy outlines payment expectations and financial responsibilities for services provided.

Payment is required at the time services are scheduled or rendered unless otherwise stated.

Accepted forms of payment may include:

• Credit card
• Debit card
• HSA/FSA cards
• Other approved electronic payment methods

By scheduling an appointment, patients agree to pay the fees associated with the services provided.

Elite Health & Hormone strives to provide transparent pricing for services. Fees for consultations, follow-up visits, laboratory services, and treatment plans will be communicated prior to the appointment whenever possible.

Additional services such as laboratory testing, medications, supplements, or specialty testing may involve additional costs.

Elite Health & Hormone operates primarily as a cash-pay practice and may not participate with insurance networks.

Patients are responsible for payment of all services regardless of insurance coverage.

If requested, we may provide documentation that patients can submit to their insurance provider for potential reimbursement. Reimbursement is not guaranteed.

Laboratory tests ordered through outside laboratories may be billed separately by the laboratory provider.

Patients are responsible for any charges associated with laboratory services.

Prescription medications are not included in consultation fees unless otherwise specified.

Patients have the right to fill prescriptions at any pharmacy of their choice.

Elite Health & Hormone may provide prescription services in accordance with the Controlled Substances Act and applicable state and federal regulations.

If Elite Health & Hormone offers membership-based care or subscription programs, the following terms may apply:

• Monthly membership fees are billed automatically
• Membership services may include consultations, care coordination, and ongoing monitoring
• Membership does not guarantee prescriptions or treatment recommendations

Membership services may be canceled according to the terms outlined at the time of enrollment.

Medical consultation services are generally non-refundable once provided.

Refunds may be considered on a case-by-case basis at the discretion of Elite Health & Hormone management.

Laboratory testing, medications, supplements, and specialty diagnostic services are typically non-refundable once ordered.

Patients who fail to attend a scheduled appointment without prior notice may be subject to a missed appointment fee.

We encourage patients to provide at least 24 hours notice if they need to cancel or reschedule an appointment.

By scheduling services with Elite Health & Hormone, patients acknowledge that they are financially responsible for all charges associated with their care.
